விருத்த சேதனம் (circumcision அரபி: ختنة; ஈப்ரு: בְּרִית מִילָה) அல்லது ஆண் விருத்த சேதனம் எனப்படுவது, ஆண்குறியின் முன் தோலை நீக்கும் சமய சடங்கு அல்லது அறுவை சிகிச்சையாகும்[1]. கூர்மையான கத்தி, கூர்தகடுகள், கவ்வி போன்றவற்றின் உதவியுடன் இது செய்யப்படுகின்றது. பெரும்பாலும் சமய மற்றும் கலாச்சார ரீதியிலும், சில இடங்களில் சுகாதார நோக்கிலும் இவை செய்யப்படுகின்றன. உலகம் முழுவதும் சுமார் 30% ஆண்களுக்கு விருத்த சேதனம் செய்யப்படுகின்றது[2]. வட கிழக்கு ஆப்பிரிக்க பழங்குடிகளிடம்தான் முதன் முதலில் விருத்த சேதனம் செய்யும் முறை நடைமுறைக்கு வந்தது. எகிப்தில் காணப்படும் பழங்கால குகை ஓவியங்களில் இருந்து இதனை அறியலாம்[3][4][5]. இதன் பின் ஆபிரகாமிய மதங்களான யூதம் மற்றும் இசுலாம் ஆகியவற்றில் புனித சடங்குகளாக இவை நடைமுறைப்படுத்தப்பட்டன. விருத்த சேதனம் பால்வினை நோய்கள், ஆண்குறி புற்றுநோய், பெண் துணையின் கர்ப்பப்பை வாய்ப் புற்றுநோய் போன்றவற்றை கட்டுப்படுத்த உதவுகின்றது. மேலும் எயிட்சு தாக்கப்படுவதற்கான இடரும் இதன் மூலம் சிறிது குறைவதாக உலக சுகாதார அமைப்பு அறிவித்துள்ளது[6][7].

வரலாறு[தொகு]

மிகப் பழமையானதாக கருதப்படும் எகிப்திய விருத்த சேதன குகை ஓவியம். கி.மு 2350 முதல் 2000க்கு இடைப்பட்டதாக இருக்கலாம்.

விருத்த சேதனம் எழுதப்பட்ட வரலாற்றுக்கு முந்தைய காலத்திலேயே வட கிழக்கு ஆப்பிரிக்க மக்களிடம் நடைமுறையில் இருந்தது[8][9]. இல்லற வாழ்வின் சுகத்தை விட்டு விலகி, கடவுளிடம் தங்களை முழுமையாக அர்ப்பணிப்பதன் அடையாளமாக இந்தச் சடங்கு நடத்தப்பட்டது. மேலும் விருத்த சேதனம் செய்யப்படுவது ஒரு ஆணின் மன இச்சையை கட்டுப்படுத்துவதாகவும், அதன் மூலம் அவனால் பாவங்களை விட்டு விலக முடியும் எனவும் அந்த மக்களால் நம்பப்பட்டது[10]. இதன் பிறகான காலகட்டத்தில் யூதர்களால் விருத்த சேதனத்துக்கு சமய அடையாளம் கொடுக்கப்பட்டது. ஆபிரகாமும் அவரது சந்ததிகளும் விருத்த சேதனம் செய்துகொள்ளுமாறு கடவுள் கட்டளையிட்டதாக யூதர்கள் நம்பினர். இதன்படி அனைத்து குழந்தைகளுக்கும் விருத்த சேதனம் செய்யப்பட்டது. இதன் தொடர்ச்சியாக தொடக்க கால கிருத்துவம், இசுலாம் ஆகியவற்றிலும் விருத்த சேதனம் நடைமுறைப்படுத்தப்பட்டது. பிற்பட்ட காலத்தில் இசுலாமியர்களின் எழுச்சி நடு கிழக்கு நாடுகள் முழுவதிலும் விருத்த சேதனத்தை அறிமுகப்படுத்தியது.

முறையே 1865, 1870 களில் யூதர்களின் மூலமாக இங்கிலாந்து மற்றும் அமெரிக்காவில் விருத்த சேதனம் அறிமுகமாகியது[11]. இதனைப் பற்றி ஆராய்ந்த நதானியேல் யாக்போர்ட் (Nathaniel heckford), லீவிசு சைர் (Lewis A. Sayre), மோசசு (M.J. Moses) போன்றவர்கள் விருத்த சேதனம் வலிப்பு, கனவு ஒழுக்கு (சொப்ன லிகதம்), கண் பார்வைக் குறைபாடு ஆகியவற்றைத் தடுப்பதோடு சுய இன்ப இச்சையையும் குறைப்பதாக அறிவித்தனர்[12]. முறையான ஆதாரங்கள் இல்லாத போதிலும், இது போன்ற கருத்துக்களைத் தொடர்ந்து பத்தொன்பதாம் நூற்றாண்டின் நடுப்பகுதி வரை விருத்த சேதனம் அமெரிக்காவில் அதிக அளவில் நடைமுறைப்படுத்தப்பட்டது. இதன் பிறகு 1971ல் அமெரிக்க குழந்தை மருத்துவக் கழகம் (American Academy of Pediatrics), விருத்த சேதனத்தால் குழந்தைகளுக்கு எந்த விதமான நன்மைகளும் கிடைக்காது என அறிவித்தது[13]. இதன் பிறகே விருத்த சேதனம் அமெரிக்கர்களிடம் செல்வாக்கை இழந்தது[14].

இதே நேரத்தில் இரண்டாம் உலகப் போர் மற்றும் கொரியப் போர் ஆகியவற்றின் மூலம் அமெரிக்க போர் வீரர்களிடம் இருந்து விருத்த சேதன முறை தென் கொரிய மக்களிடமும் பிரபலமானது[15].

யூதம்[தொகு]

மித்வா அசா என்ற பெயரில் யூதர்களால் விருத்த சேதனம் அழைக்கப்படுகின்றது. ஏறக்குறைய கிமு 600 முதல் விருத்த சேதனம் செய்து கொள்ளும் பழக்கம் யூதர்களிடம் உள்ளதாக நம்பப்படுகின்றது. கடவுளின் கட்டளைப்படி ஆபிரகாமும் அவரின் புதல்வர்களும் விருத்த சேதனம் செய்து கொண்டதாக ஆதியாகமம் கூறுகின்றது[16][17]. இதன் அடிப்படையில் ஒவ்வொரு யூதக் குழந்தைக்கும் பிறந்த எட்டாம் நாள் விருத்த சேதனம் செய்யப்படுகின்றது. மோகள் எனப்படும் மத நிபுணர்கள் இந்த சடங்கை நடத்தி வைக்கின்றனர். பிறித் மிழா என்ற பெயரில் ஒரு விழாவாக இந்த சடங்கு யூதர்களால் நடத்தப்படுகின்றது.

கிறித்தவம்[தொகு]

கிறித்தவம் தொடக்கத்தில் தன்னை யூத மதத்தின் ஒரு சீர்திருத்தப் பிரிவாகவே கருதியதாலும் அதன் முதல் உறுப்பினர்கள் அனைவரும் யூதர்களாக இருந்ததாலும், விருத்த சேதனம் செய்து கொள்ளும் பழக்கம் அவர்களிடையே இருந்தது[18]. யூதரான இயேசு கிறித்துவுக்கும் யூத மரபு படி பிறந்த எட்டாம் நாள் விருத்த சேதனம் செய்யப்பட்டது[19]. இருப்பினும் இயேசுவின் உயிர்த்தெழுதலுக்குப் பின் யூதேயாவிலிருந்த சிலர், மோசேயின் முறைமைப்படி விருத்தசேதனம் செய்து கொள்ளாவிட்டால் மீட்படைய முடியாது என்று கற்பித்து வந்தனர். அவர்களுக்கும் பவுல், பர்னபா ஆகியோருக்குமிடையே பெருங் கருத்து வேறுபாடும் விவாதமும் உண்டாயின. எனவே பவுலும் பர்னபாவும் ஏறக்குறைய கிபி 50ல் எருசலேமுக்குச் சென்று, திருத்தூதர்களிடமும் மூப்பர்களிடமும் இந்த சிக்கலைக் குறித்துக் கலந்து பேசுமாறு சங்கமொன்றைக்கூட்டினர். இதில் இன்றியமையாதவற்றைத் தவிர அதிகமான வேறு எந்தச் சுமையையும் இருக்கக்கூடாது என தீர்மானிக்கப்பட்டது. ஆகவே விருத்த சேதனம் தேவையற்றது என உறுதி செய்யப்பட்டது[20]. இது முதல் திருத்தந்தையான பேதுருவின் ஒப்புதலைப்பெற்றிருந்ததால்[21][22][23] விருத்த சேதனம் கிறித்தவர்களிடம் இல்லாமல் போனது.

இருப்பினும் இன்றும் ஆப்பிரிக்காவில் உள்ள சில கிழக்கு மரபு வழி காப்திக், எத்தியோப்பியன் மற்றும் எறித்தீன் ஆகிய பிரிவுகளில் விருத்த சேதனம் வழமையான ஒன்றாகவே உள்ளது[24]. குறிப்பாக கென்யாவில் உள்ள நோமியா பிரிவு போன்றவை, தனது உறுப்பினர்களுக்கு விருத்த சேதனத்தை கட்டாயமாக்கி உள்ளது[25].

இசுலாம்[தொகு]

இசுலாமில் கத்னா, சுன்னத் போன்ற பெயர்களில் விருத்த சேதனம் அழைக்கப்படுகின்றது. இது ஒரு கடமையாக குரானில் நேரடியாக அறிவிக்கப்படாத போதிலும், நபிவழி என்ற முறையில் உலகம் முழுவதிலும் இது கடைபிடிக்கப் படுகின்றது[26]. விருத்த சேதனம் செய்து கொள்ளுவது சுகாதாரமான முறை என முகம்மது அறிவித்ததாக பல நபிமொழி நூல்கள் கூறுகின்றன[27]. இன்னும் சிலர் முகம்மது குழந்தையாக பிறக்கும் போதே விருத்த சேதனம் செய்யப்பட்டவராக பிறந்தார் என அறிவிக்கின்றனர். ஆனால் இதற்கு எந்தவிதமான ஆதாரமும் இல்லை.

யூதர்களைப் போல அல்லமால் இசுலாமியர்கள் சிறு மற்றும் பருவ வயதிலேயே விருத்த சேதனம் செய்து கொள்கின்றனர். சில நடு கிழக்கு நாடுகளில் மட்டும் குழந்தை பிறந்த ஏழு அல்லது நாற்பதாவது நாளிலேயே விருத்த சேதனம் செய்யப்படுகின்றது. பெரும்பாலும் எளிமையான முறையிலேயே இந்த சடங்கு நடத்தப்பட்டாலும், துருக்கி மற்றும் தெற்கு ஆசிய இசுலாமியர்களிடையே இந்த சடங்கு ஒரு சுபவிழாவாக நடத்தப்படுகின்றது. உலகம் முழுவதும் விருத்த சேதனம் செய்து கொள்ளும் ஆண்களில் 68% பேர் இசுலாமியர்கள் ஆவர்[28].

கலாசாரம்[தொகு]

மதக் கடமை என்ற முறையில் மட்டும் அல்லாமல் கலாச்சார முறையிலும் சில இடங்களில் விருத்த சேதனம் கடைபிடிக்கப்படுகின்றது. குறிப்பாக நைசீரியாவில் உள்ள சில குழுக்கள், அற்கம் லேன்ட், அவுத்திரேலியாவின் மத்திய மற்றும் மேற்கு பாலைவனங்களில் உள்ள பழங்குடிகள் ஆகியோரிடமும் விருத்த சேதன பழக்கம் உள்ளது[29][30].

மேலும் பசுபிக் தீவுக் கூட்டங்களான பிசிய், வனுவாட்டு, பெந்தேகோசுடு தீவுகள் மற்றும் பாலினேசியன் தீவுகுளான சமோவா, தொங்கா, குவாம், வடக்கு மரியானா தீவுகள், நியுவே, திகோபியா ஆகியவற்றில் உள்ள மக்களாலும் விருத்த சேதனம் கடைப் பிடக்கப்படுகின்றது. ஆணில் இருந்து பெண் தன்மையை நீக்கும் நிகழ்வாக விருத்த சேதனம் மேற்கு ஆப்பிரிக்க தோகன் மக்களால் கருதப்படுகின்றது[31].

மதுரை வட்டாரத்திலுள்ள 'பிறமலைக்கள்ளர்' சமூகத்தில் விருத்த சேதனம் 'கவரடைப்பு' அல்லது 'கவரடப்பு' என்ற பெயரில் கடைபிடிக்கப்படுகிறது. தற்காலத்தில் இது 'மார்க்கல்யாணம்' என வழங்கப்பெறுகிறது. தமிழ்ச் சமுதாயத்தில் பல சமூகங்களில் இருந்த இந்தப் பழக்கம், பிரமலைக் கள்ளர்களிடம் மட்டும்தான் எஞ்சியுள்ளது.[32].

பொதுவாக இசுலாமியர்கள் அதிகம் வாழும் நடு கிழக்கு மற்றும் ஆப்பிரிக்க நாடுகளிலும், இசுரேலிலும் அதிக அளவில் விருத்த சேதனம் செய்யப்படுகின்றது. 2006ம் ஆண்டு உலக சுகாதார அமைப்பு வெளியிட்ட ஒரு அறிக்கை, உலக அளவில் 30 முதல் 33 சதவிகித ஆண்கள் விருத்த சேதனம் செய்து கொள்வதாக கூறுகின்றது. மேலும் இதன் கணக்கீடுகள் விருத்த சேதன பழக்கமானது கனடா, அவுத்திரேலியா, இங்கிலாந்து ஆகிய நாடுகளில் இறங்கு முகமாகவும்[33], தென் ஆப்பிரிக்காவில் ஏறு முகமாகவும்[34] உள்ளதை சுட்டிக்காட்டுகின்றது. நாடுகள் வாரியாக விருத்த சேதனத்தின் பரவல் பின் வருமாறு உள்ளது[33].

மத ரீதியாக செய்யப்படும் விருத்த சேதனங்களுக்கு எதிர்ப்புகள் இல்லை என்ற போதிலும், சுகாதார நோக்கோடு செய்யப்படும் குழந்தை விருத்த சேதனங்களுக்கு பரவலாக எதிர்ப்புகள் எழுந்தவண்ணம் உள்ளன. குறிப்பாக இங்கிலாந்து மற்றும் அமெரிக்காவில் இவ்வகையான எதிர்ப்புகள் அதிக அளவில் உள்ளன. குழந்தைகளுக்கு செய்யப்படும் விருத்த சேதனமும் ஒரு வகையான மனித உரிமை மீறலே என்பது இவர்களின் வாதம்[35][36][37]. பிரிட்டிசு மருத்துவக் கழகம் (British Medical Association) மற்றும் ராயல் அசுத்ரேலியன் மருத்துவக் கல்லூரி (Royal Australasian College of Physicians) போன்றவையும் இவர்களை ஆதரிக்கின்றன[38][39]. குழந்தைக்கு தேவையானதை பெற்றோர்களே முடிவு செய்ய முடியும் என்ற போதிலும், அவர்களின் மீதான உரிமைகள் ஒரு கட்டுப்படுத்தப்பட்ட அளவிலே இருக்க வேண்டும் என்பது இவர்களின் வாதம் ஆகும். இருப்பினும் பெரும்பாலான மருத்துவக் கழகங்கள் குழந்தை விருத்த சேதன முறையை ஆதரிக்கவே செய்கின்றன[40][41][42].

பாலியல் தாக்கங்கள்[தொகு]

ஆண்கள்[தொகு]

விருத்த சேதன முறை எதிர் மறையான பாலியல் தாக்கங்களை உருவாக்கும் என்பது எதிர்ப்பாளர்களின் கருத்து. ஆணுறுப்பு எழுச்சி, தூண்டல், செயல்பாடு ஆகியவற்றில் இது மந்த நிலையை உருவாக்கும் என்பது இவர்களின் வாதம். ஆனால் இதைப் பற்றிய கருத்துக் கணிப்புகள் இதை மறுக்கின்றன. புதிதாக விருத்த சேதனம் செய்து கொண்ட 150 இங்கிலாந்து ஆண்களிடம் மேற்கொள்ளப்பட்ட ஒரு ஆய்வு, விருத்த சேதனம் குறிப்பிடத்தக்க அளவில் எந்த விதமான பாலியல் விளைவையும் ஏற்படுத்துவதில்லை எனக் கூறுகின்றது[43]. இதில் பங்குகொண்ட ஆண்களில் 38% பேர் விருத்த சேதனம் தங்களுக்கு உறவின் பொழுது அதிக கிளர்ச்சியை உண்டாக்குவதாகவும், 18% பேர் அதை குறைப்பதாகவும், 44% பேர் எந்தவிதமான மாற்றமும் இல்லை எனவும் கருத்து தெரிவித்துள்ளனர். மாறாக 18 மற்றும் 20 வயதுக்கு மேல் விருத்த சேதனம் செய்து கொண்ட 255 கொரிய ஆண்களிடம் நடத்தப் பட்ட ஒரு கருத்துக் கணிப்பு, விருத்த சேதனம் சுய இன்ப சுகத்தை குறைப்பதாகவும், அதன் உச்சத்தை அடைவதை சற்று கடினமாக்குவதாகவும் கூறுகின்றது[44]. 48% பேர் இதையொட்டியும் 8% பேர் இதற்கு எதிராகவும் கருத்து தெரிவித்துள்ளனர்.

இருப்பினும் விருத்த சேதனம் பாலியல் ரீதியாக குறிப்பிடத்தக்க தாக்கங்களை ஏற்படுத்துவதில்லை என்பதே உலகின் பல பகுதிகளில் மேற்கொள்ளப்பட்ட ஆய்வுகளின் கருத்தாகும்[45][46][47][48][49].

பெண்கள்[தொகு]

யோனி உலர்வு பிரச்சனை உள்ள பெண்கள் விருத்த சேதனம் செய்துகொண்ட ஆண்களுடன் புணர்ச்சியில் ஈடுபடுவது சில அசெளகரியங்களை உண்டாக்குகின்றது. 2003ல் பென்சுலே மற்றும் பாயில் ஆகிய உளவியளாலர்கள் வெளியிட்ட கூட்டறிக்கை இதை உறுதிப்படுத்துகின்றது[50]. விருத்த சேதனம் செய்து கொண்ட ஆண் துணையை உடைய 19 பெண்களிடம் கார்ட்டசு-கோன்சலசு நடத்திய ஆய்வில் குறிப்பிடத்தக்க அளவு ஈரப்பசைக் குறைவு ஏற்படுவது கண்டுபிடிக்கப்படட்து[51]. இருப்பினும் இது எந்த விதத்திலும் தங்களின் புணர்ச்சிக்கும், சுகத்திற்கும் தடையாகவோ அல்லது வலி ஏற்படுத்துவதாகவோ இல்லை என்று பங்குகொண்ட பெண்கள் தெரிவித்துள்ளனர். மேலும் 455 ஆப்பிரிக்க பெண்களிடம் கிகோசி என்பவர் மேற்கொண்ட ஆய்வின் முடிவில், 39.8% பெண்கள் விருதத சேதனம் செய்து கொண்ட ஆண்களால் புணர்ச்சி சுகம் அதிகரிப்பதாகவும், 2.9% பெண்கள் சுகம் குறைவதாகவும், 57.3% பெண்கள் குறிப்பிடத்தக்க வேறுபாடு இல்லை எனவும் கருத்து தெரிவித்துள்ளனர்[52].

இதே போல் 1988ல் வில்லியம்சன் என்பவர் அமெரிக்காவின் அயோவா மாநிலத்தில் நடத்திய ஆய்வில் 76% பெண்களும், 1976ல் வைல்ட்மேன் என்பவர் சியார்சியா மாநிலத்தில் நடத்திய ஆய்வில் 89% பெண்களும் விருத்த சேதனம் செய்துகொண்ட ஆண்களை விரும்புவதாக கருத்து தெரிவித்துள்ளனர்[53][54].

ஆய்வுகள்[தொகு]

ஏப்ரல் 16, 2013 அன்று வெளியிடப்பட்ட எம்பையோவின் அறிக்கையில், ஆண்குறியின் முன்தோலை நீக்கிய பின், பெரும்பாலான உயிர்வளிவேண்டா பாக்டீரியா அந்த இடத்தில் உயிர்வாயுக்கள் கிடைக்கபெறுவதாய் உள்ளதினால் விருத்த சேதனம் செய்யப்படாத ஆண்குறியைக் காட்டிலும் குறைவான உயிர்வளிவேண்டா பாக்டீரியா காணப்படுவதாக கூறப்பட்டுள்ளது.[55] ஆனால் இதுபோல் முன்தோல் நீக்கப்படுவதினால் கெச்சு.ஐ.வி பரவல் அதிகமாகாது என்று சொல்வதற்கில்லை என்றும், இது அதற்கு மாறுபட்ட ஆய்வு எனவும் சிகாகோவில் உள்ள இல்லினாய்சு பல்கலைக்கழகத்தின் உயிரியலாளர் மற்றும் கொள்ளை நோயியலாளரான ரோபர்டு பைலேய் (Robert Bailey) கூறுகிறார்.[56]

The review led us to conclude the following. There is evidence that circumcision results in an approximately 12-fold reduction in the incidence of UTI during infancy. The overall incidence of UTI in male infants appears to be 1% to 2%. The incidence rate of the complications of circumcision reported in published articles varies, but it is generally in the order of 0.2% to 2%. Most complications are minor, but occasionally serious complications occur. There is a need for good epidemiological data on the incidence of the surgical complications of circumcision, of the later complications of circumcision and of problems associated with lack of circumcision. Evaluation of alternative methods of preventing UTI in infancy is required. More information on the effect of simple hygienic interventions is needed. Information is required on the incidence of circumcision that is truly needed in later childhood. There is evidence that circumcision results in a reduction in the incidence of penile cancer and of HIV transmission. However, there is inadequate information to recommend circumcision as a public health measure to prevent these diseases. When circumcision is performed, appropriate attention needs to be paid to pain relief. The overall evidence of the benefits and harms of circumcision is so evenly balanced that it does not support recommending circumcision as a routine procedure for newborns. There is therefore no indication that the position taken by the CPS in 1982 should be changed. When parents are making a decision about circumcision, they should be advised of the present state of medical knowledge about its benefits and harms. Their decision may ultimately be based on personal, religious or cultural factors."
